Direct Primary Care is a subscription-based model that places your relationship with your doctor at the center of your care.

per month, or $594 annually
For children between the ages of 0 to 17. *Children require a paying adult membership.

✓ Direct access to your doctor (via text, email, phone, in office, telehealth).
✓ Sick visits for infections, injuries, rashes, etc.
✓ Sports Physicals, Wellness Visits and Screenings.
✓ Procedures, as needed.

$105
per month, or $1134 annually
For adults aged of 18 to 45.

MEMBERSHIP TERMS:
*A 10% discount is applied to the total fee for memberships pre-paid annually.
*Family maximum for at least one paying adult with dependents residing at the same physical address is $325/month.
*For monthly billing, we require a minimum three month commitment at the time of enrollment. If you choose to cancel after this time, no hard feelings. And if you cancel, then change your mind more than 30 days later, you can re-enroll (just once) after paying a $250 re-enrollment fee. But we’re betting you won’t cancel once you see the value our care adds to your life.
